Family Case Management

The Family Case Management Program (FCM) is a statewide program that helps income eligible pregnant woman and infants to obtain the health care services they may need to have a healthy pregnancy and to promote the infant's healthy development.

Southern Seven Health Department ensures that all of our clients get connected to the resources they need for optimal health.  Our public health team can provide you with referrals to other agencies such as Department of Human Services, Housing, Mental Health counseling, etc.  Contact us if you would like more information on this service.
